One-Year Cardiovascular Event Rates in Outpatients With Atherothrombosis

This manuscript confirms that PAD is such a major predictor of future cardiovascular, peripheral vascular and cerebrovascular events that every effort should be made to identify it even in asymptomatic patients. The data comes from the one year review of the REACH registry, a global study of 68,236 outpatients from 44 countries. The incidence of cardiovascular death, MI, stroke or hospitalization for atherothrombotic events after one year for CAD was 15.1%, for CVD 14.3% and for PAD 21.1%. The manuscript is replete with data about these patient populations and includes information on patterns of medication and diagnosis. However, the take home message is that identifying concomitant PAD should be a priority, since its presence mandates implementation of intensive risk factor modification. Unfortunately, Medicare does not pay for screening asymptomatic patients.
